- Active symptom monitoring: Care visitors are prompted to confirm if any symptom that was present on the last visit is present on this visit. This provides a consistency and frequency to symptom monitoring.
- Raising a concern on visit: This is a digital process for care visitors to raise a concern on the platform. This is delivered directly to the dashboard users for remote review and action (Such as to create a priority observation or to log a referral).
- Priority observation monitoring: Priority observations are set by health professionals or senior social care professionals. These are highlighted within the visit app for the care visitor to record if present. This is mostly used to monitor key symptom when awaiting an intervention or if more information is needed after a concern has been raised.
- Recording in/dependency scores with activities of daily living (ADL): Codesigned with a reablement team, recording level of support needed for ADLs provides a consistent measure that can be reviewed in line with other signs and symptoms recorded to quickly identify any developing or decreasing needs.
- Referral management: Health care referrals logged on the platform and can be tracked in line with symptom progression to manage risk. Any engagement with referrals is recorded in an audit log which is visible to patients, families and social care professionals.
- Automatic Alerts: These rule-based alerts trigger notifications to key services and disciplines when certain information is recorded in the visit app. An example of these are safeguarding alerts, mental health alerts and Palliative – advanced care planning alerts.
